Submitting an accurate blue emergency contact card is crucial for your child’s safety. It ensures the school can quickly reach you or designated contacts in case of an emergency. Keeping this information up-to-date is essential for effective communication and your child’s well-being while at school.

The walking trip form for students in grades 3K to 5 allows the school to take children on nearby educational outings, such as visits to local parks, libraries, or cultural sites. By signing this form, parents give permission for their child to participate in these supervised, enriching experiences that enhance classroom learning.​

This form covers school-sponsored or other DOE-sponsored events only. The form covers both recordings of students (for example filming, photo-taking, and audio recording) made by the school or others at the DOE, together with press or others reporting on the event.

In addition to academic support, students have access to speech and language pathology, occupational therapy, and physical therapy. These services are integrated into their daily routines to address their holistic developmental needs.

Early arrival at 7:45 AM at P.S. 145 is designed to support working parents by providing a safe and supervised environment for their children before the school day begins. This program ensures that students can start their day on time, while parents can manage their work schedules more effectively.

If you are seeking a student MetroCard, please read the requirements and fill out the form below.
1. Students only in grades K - 5 qualify.
2. You must have your correct address registered at the school.
3. You must live at least 0.5 miles away.
4. You must not have yellow school bus services, or have applied to the yellow school bus services. Your child may not have a MetroCard and have yellow bus service.
If you meet these requirements, students name/ID will be submitted to Office of Pupil Transportation for final approval. Once approved, we will email you that the card is being backpacked with your child.

The parent coordinator intake form allows parents to easily request an appointment online to discuss various concerns, including afterschool programming, lunchtime incidents, possible bullying, busing issues, or MetroCard needs. By completing this form, parents can ensure their concerns are addressed promptly and efficiently by the appropriate staff member.
